Job Description:
For more than 55 years, Family Health Centers of San Diego’s
(FHCSD) mission has been to provide caring, affordable,
high-quality health care and supportive services to everyone, with
a special commitment to uninsured, low-income and medically
underserved persons. FHCSD is one of the top 10 largest federally
qualified health centers (FQHCs) in the country. We operate more
than 90 sites across San Diego County, including 29 primary care
clinics, 23 behavioral health facilities, 10 physical
rehabilitation clinics, nine dental clinics, five vision clinics,
four outpatient substance use treatment programs, three mobile
medical units, two mobile counseling centers, two urgent care
centers, and a pharmacy. Our staff provides care to over 227,000
patients each year, of whom 91% are low-income and 29% are
uninsured. FHCSD provides care to all. Services include, but are
not limited to adult care, chronic disease management, pediatrics,
comprehensive women’s care including obstetrics, dental, vision,
case management, physical rehabilitation, speech therapy for
children, vaccinations, infectious diseases, behavioral health,
substance use counseling and a host of specialty services including
cardiology, podiatry, endocrinology, dermatology, among others.
FHCSD also offers supportive services to those who are unsheltered
and in need of intensive case management. The breadth of our clinic
locations, services and programs has grown over the last five
decades, making us the largest community clinic provider of health
care to the uninsured in the county and one of the top 10 largest
community clinic organizations in the nation. We are also the
largest health care safety-net provider, largest school-based
health care provider and the largest mental health provider in the
San Diego region. Responsibilities: Orient all new department staff
by presenting various forms of onboarding, orientation, and skills
training. Provide training on ECM program requirements, virtual
platforms and any program updates. Train all staff on FHCSD
documentation standards. Function as a liaison between FHCSD and
Medi-Cal managed care plans for any required electronic portal
documentation changes and regulations. Create Enhanced Care
Management FHCSD Training Manual, and update any relevant material.
Present training and development programs using various forms and
formats to include lecture and videos. Create and maintain training
calendars across multiple departments. Develop a calendar of annual
training requirements for all employees. Conduct extensive training
for Lead Care Managers and Community Health Specialists on work
protocols, serving as a subject matter expert. On an as needed
basis, provide coaching, mentoring, and input on performance
reviews. Maintain knowledge of new methods and techniques for
training, and training requirements applicable to program. Complete
all required documentation accurately, in a timely manner, and
thoroughly in accordance with both state and department standards.
Assist in preparing special reports for management on an as-needed
basis. Assist management in developing and implementing work
improvements through identification of problems and opportunities
that training could improve. Maintain knowledge of the latest
trends in training and development. Evaluate program effectiveness
through assessments, surveys, and feedback. Provide input to
management in developing measurable goals/performance standards.
May carry a caseload of up to 10 patents to help support ongoing
ECM efforts. Perform other duties as assigned. Requirements:
Bachelor's degree in social work, public health, science, or other
related degree required. Or equivalent combination of education and
experience that provides the skills, knowledge and ability to
perform the essential job duties, and which meets any required
state or federal certification requirements. Traveling between
sites and other locations is required as an essential function of
the job. Must have a car, a valid California driver’s license, and
proof of minimum levels of car insurance as required under
California law, although limits of $100,000 are recommended. An
acceptable driving record is also required. California law requires
all drivers to obtain a valid California driver’s license within
ten days of establishing residency. Mileage and other reimbursement
governed by policy. 2 – 4 years of experience with health education
or working in the medical/health related field. 1 - 2 years of
experience with training in EMR healthcare training, adult
curriculum design or related experience preferred. Ability to
exercise discretion and maintain confidential information. Ability
to relate well with team members, coworkers, and other departmental
staff. Knowledge of adult health coaching and outreach techniques,
best practices, and tools. Knowledge of current health science
theories and best practices, and ability to translate best
practices into meaningful, culturally appropriate resources. Strong
documentation skills in computer applications and software. Strong
verbal and written communication skills, including the ability to
communicate effectively with those who speak English as a second
language. Demonstrated understanding of virtual training and
communication platforms (Zoom, Teams, WebEx, etc.). Strong
organization skills with the ability to adjust priorities as
needed. Strong PowerPoint skills and proficient in Microsoft
