Physical -Therapist -Job -Description/Requirements:
Evaluation/Treatment -of -Patients -in -their -home. -Driving -is
-involved -in -this -position.
Assist -in -evaluating -the -level -of -function -and -in
-developing -the -plan -of -care.
Evaluate -the -home -environment -and -makes -appropriate
-recommendations
This is an independent contractor position and the -pay is $85-115
per -visit. -Potential -to -make -$150,000- -180,000 -yearly.

-Burbank, -Diamond -Bar, -West -Covina, -arcadia.
Riverside County, San Bernardino County, Orange County: -Rancho
-Cucamonga, -Chino, -Diamond -Bar, -Rialto, -Riverside, -Colton,
-Moreno -Valley, -Temecula, -Corona, -Huntington -Beach, -Santa
-Ana, -Anaheim, -Newport.

Minimum -Required -Qualifications:
Current -CA -Physical -Therapy -(PT) -License
Valid -California -Driver's -License/ -with -transportation
-insurance
