- On-site position
- Monday- Friday, 8am-5pm
- Responsible for managing patient care and treatment (in person, telephonically and/or video) in collaboration with the physician.
- Perform health assessments and preventative health measures within prescribed guidelines and physician instructions.
- Order, interpret, and evaluate diagnostic tests to identify and assess patients clinical problems and health care needs.
- Discusses case with physician.
- Formulate and document care plan.
- Prescribe medication or other forms of treatment.
- Provide weekend telephone coverage once-twice per quarter
- Avg 12-14 patients per day: 30 minutes for established patients; 45 minutes for new patients
Minimum Requirements:
- Current unrestricted RN license and NP license in California required.
- Active and unrestricted DEA certification required.
- AANP or ANCC board certification required.
- BLS certification required.
Preferred Qualifications:
- Bilingual Spanish speaking
- Complex, chronic care, or geriatrics experience
