Job Description:

**OVERVIEW OF POSITION:**Responsible for developing and delivering the outpatient nutritional program.**ESSENTIAL FUNCTIONS:**+ Consistently exhibits behavior and communication skills that demonstrate HealthCare Partners' (HCP) commitment to superior customer service, including quality, care and concern with each and every internal and external customer.+ Participates as a member of the inter-disciplinary healthcare team and facilitates the continuum of care and education of patients that are referred for nutrition consultations.+ Counsels and educates individual patients on general and specific nutrition care in conjunction with their medical clinician and provides follow-up data.+ Evaluates food diaries to assure the proper transfer of knowledge of diet instruction and menu planning and to use as a tool to monitor dietary habits and food preferences.+ Ensures correct and timely documentation in the patient's medical record following individual consultations.+ Develops and conducts group presentations and related health education programs as needed.+ Develops and maintains patient education materials relating to general and specific nutrition needs of various populations.+ Serves as a resource for clinic staff, home health, care management and other departments.+ Provides exercise/activity information and methods in developing a safe and consistent exercise program.+ Completes patient registration form for classes.+ Collects individual fees for classes.+ Reviews current literature and resources for relevance to contribute to changes and improvements.+ Maintains current ADA registration, CPR and other appropriate licensures/certifications.+ Maintains, as needed, systems for tracking/reporting utilization and reports them in a timely manner.+ Charts/documents consultations on the appropriate progress notes.+ Provides/conducts telephonic nutrition consultations with needed follow-up.+ Communicates important patient information regarding progress to clinicians.+ Supports the Marketing department by preparing and conducting nutrition and health presentations for various employer groups/health plans.+ Maintains database and tracking system of patients in classes and applies appropriate metrics to determine outcomes and end results. Reports data as required.+ Conducts and facilitates weight management maintenance groups.+ Demonstrates the appropriate communication and teaching skills for various age groups.+ Assesses age-related patient data in order to provide the appropriate learning resources and to address specific learning needs of patients.+ Collaborates with the clinician in referring patient to other health professionals, as needed.+ Assists patient in developing problem-solving skills in dietary and lifestyle management.+ Provides individualized nutrition prescription utilizing the appropriate dietary planning approach.+ Utilizes a variety of nutrition interventions and behavior management approaches.+ Uses, protects, and discloses HCP patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.+ Performs additional duties as assigned.**EDUCATION:**+ Bachelor's degree from a four-year college and/or a professional certification requiring formal education beyond a two-year college or equivalent experience.+ Registered Dietitian by the American Dietetics Association.+ Bachelor's degree in Nutrient/Dietetics preferred.**EXPERIENCE:****_Minimum:_**+ Over 3 years and up to and including 5 years of experience.**_Preferred:_**+ 2 to 3 years experience in a hospital or outpatient setting.**KNOWLEDGE, SKILLS, ABILITIES:**+ Computer literate.+ Effective communication skills.+ Teaching skills.UnitedHealth Group requires all new hires and employees to report their COVID-19 vaccination status+ **COVID-19 Work Location Requirement:** You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.**Careers with Optum.** Here's the idea.
